Safety Bulletin for SNORKEL - AB46JE, A46JE, AB46JRT and A46JRT machines

The purpose for this SAFETY RELATED BULLETIN is to inspect and ensure the Slave Level Stepped Pins SOCKET HEAD CAPSCREW are torqued properly. Failure to comply with this SAFETY RELATED BULLETIN could result in DEATH or SERIOUS INJURY.

This inspection should take approximately 1 hour. During the inspection process, if you determine the bolts are not properly torqued, please contact the Snorkel Service Department with the model, complete serial number and hour meter reading.

Italmec Model Ital Jib 42N - Jib failure

An operator was killed when the jib supporting the bucket of the EWP he was operating failed, causing him to fall approximately 20 metres with the bucket. The EWP was an Italmec model Ital Jib 42N. The failure occurred between the jib luffing ram attachment points.
